Advanced Directives
The most recent copy of the Advanced Directives Information Packet is available by clicking the link below. Given the size of the document (36 pages) and depending on your connection speed, the packet may take a few minutes to download. The packet includes information and forms for Healthcare Representatives, Living Will Declaration, Life-Prolonging Procedures Declaration, Out of Hospital "Do Not Resuscitate," Organ and Tissue Donation, Healthcare Treatment Directive, Psychiatric Advance Directive and Power of Attorney.
Advance Directives
Birth Certficates - Certified copies
Many entities require the presentation of a certified copy of birth certificates which are the official records of birth. These are not maintained by the Hospitals of birth. Certified copies of birth and death certificates are available from the Indiana State Department of Health (ISDH) or available from the local health department in the county where the event occurred. All requests require proper identification.

Insurance Programs
The Healthy Indiana Plan
The Healthy Indiana Plan (HIP) is a new, affordable health insurance program for uninsured adult Hoosiers. The program is sponsored by the State and only requires minimal monthly contributions from the participant. The Pre-Existing Condition Insurance Plan
If you are uninsured, having a hard time finding health coverage and have been turned down for health coverage because of a pre-existing condition this may be for you. The Pre-Existing Condition Insurance Plan is a relatively new, affordable Federal health insurance program for uninsured adults.
