Not every person will need the same screenings or follow-up care. Our team will help guide you toward the recommendations that are right for you.

*Must be 18 years of age to participate in the FREE screening and door prize drawings. Fasting is not required, but some may prefer to.

After You Submit the Registration Request Form:

• • A limited number of appointments are available per timeslot
  • You’ll receive a confirmation email within 3–4 business days if your preferred time is available
    • The messages you receive are sent from “Decatur County Memorial Hospital via Smartsheet“
  • If your preferred time is full, DCMH Registration will contact you to reschedule
  • You must receive a confirmation email with additional information to be officially registered
  • Requests submitted after hours, on weekends, or holidays will not be addressed until the next business day
  • Note: Requests are processed in the order received, and response times may vary depending on volume

1. Who is eligible for the free screenings?
All community members 18 years of age and older are eligible

• • DCMH Full-Time, Part-Time, PRN, and Contract employees are not eligible

2. How do I register?
Use the registration form on the Men’s Health Tailgate page to schedule your appointment

3. Will I receive a confirmation message?
Yes. Once your appointment is confirmed, you will receive a confirmation email with your appointment date, time, and location. You’ll also receive a reminder email one day before your appointment with parking and check-in details.

4. Where do I go for my appointment?
Location details are included in your confirmation email

5. Do I need to fast for the lab screening?
Fasting is not required, though some may prefer to do so

6. How do I cancel my appointment if I can no longer attend?
Reply to any confirmation or reminder email with “CANCEL” so your spot can be offered to someone on the waitlist

7. How do I receive my results?
Your results will not be mailed.

• • All results will be available in your Patient Portal within 4 weeks of the event: dcmh.net/patient-portal

  • If you do not have a Patient Portal account already, you will receive an invitation email within 5 business days of the event

  • If you have questions about accessing your Patient Portal, please visit this page or contact DCMH Registration at 812-663-1149 or regrequest@dcmh.net

  • Critical results and follow-up recommendations will be communicated by a phone call within 4 weeks of the event where you can schedule any necessary appointments

  • Paper copies of results are available upon request through DCMH’s Health Information Management (HIM)/Medical Records department within 4 weeks of the event
