For Amy Prentice, RN, BSN, care isn’t just something she provides; it’s something she makes people feel. And for one Franklin County resident, that feeling has made all the difference.
Living with her daughter and great-granddaughter, Joyce Stevens stays busy with the things she loves: crocheting, gardening, learning to knit, and spending time cooking and baking. Life is full, but managing health between appointments used to feel uncertain at times.
That’s where Decatur County Memorial Hospital’s Connected Care program stepped in.
Connected Care provides ongoing support for individuals managing chronic conditions, offering direct access to a care team between visits. For Joyce, that support comes in the form of a simple, consistent connection through a call or a text.
“Amy is always there, she’s kind, sweet, thoughtful, and she really cares about me,” Joyce shared. “I’ve worked with her for several years, and I don’t know why you wouldn’t want to get help from someone like her.”
Amy is there each step of the way by answering questions, helping coordinate care, scheduling lab appointments, recommending next steps like connecting her with a new family medicine provider, and even sharing helpful tips about heart health month.
“She helps with everything I need,” Joyce said. “I can text or call her anytime.”
That accessibility has made a meaningful impact, not just for Joyce, but for her family as well. “I’m happy knowing someone else is checking in too,” her daughter shared. “It gives us peace of mind.”
But what stands out most is how personal the experience feels.
“I know the hospital has a lot of patients,” Joyce said, “but I always feel like I’m the only one. In every conversation with Amy and in each department in the hospital, I feel like a special person.”
That feeling of being seen, heard, and supported has helped Joyce stay on track with her health while continuing to enjoy the life she’s built, whether that’s spending time with family or working on her latest craft project.
Connected Care ensures that patients don’t have to wait for their next appointment to get the support they need. With a dedicated care team just a call or text away, help is within reach.
“I’m proud to be part of our community and to connect with patients in a real, meaningful way,” said Amy. “I enjoy getting to know them and helping them feel supported and confident as they move forward.”
